Privacy Policy

This privacy policy sets out how ‘Gray’s Nutrition’ uses and protects any personal information that you provide us.

‘Gray’s Nutrition’ is committed to ensuring that your privacy is protected. Should we ask you to provide certain information by which you can be identified when using our services, you can be assured that it will only be used in accordance with this privacy policy.

This policy is effective as from 23rd May 2018.

Why we collect your data:
We collect your personal data, because you give us consent to do so, in order to provide our independent nutrition and dietetic services to you.

Who has access to your data:
  • Ruth Harvey (Registered Dietitian) is the only person with access to your diet and medical data.
  • Holmedale Health, St Leonards, Exeter – if you have been seen by Ruth Harvey at Holmedale Health then they will have a record of this visit and your name and contact details on file.  Please contact them to request their privacy policy or request data to be deleted.

What we collect:
We may collect the following information:
  • Information in relation to the consent form (as required if you agree to receiving services from Gray's Nutrition);
  • Further clinical information in relation to reports from other health professionals.

What we do with the information we gather:
We require this information for the purpose of:
  • Professional clinical record keeping of client information;
  • Sharing information with relevant health professionals when client permission granted.

We are committed to ensuring that your information is secure. In order to prevent unauthorised access or disclosure, we have put in place encrypted electronic systems and standard operating procedures to safeguard and secure the information we collect.

Controlling your personal information
We will not distribute, sell or lease your personal information to third parties unless we have your explicit permission or are required by law to do so. You may request details of personal information, which we hold about you under the General Data Protection Regulation (GDPR).  If you would like a copy of the information held on you please contact Ruth Harvey at [email protected]

If you believe that any information we are holding on you is incorrect or incomplete, please email or write to us as soon as possible. We will promptly correct any information found to be incorrect.

How you can withdraw and request to be deleted from our files
I am required by law to hold medical records for at least 8 years after which time they will be deleted or shredded according to their format.  If you have consented for Gray’s Nutrition to collect your data this means you have consented for your records to be held for this period of time.  If you have not consented then we will dispose of any records after 1 year unless instructed otherwise.

Paper records are kept in a locked filing cabinet, whilst online records are kept on the GDPR compliant site HealthKit.com. Under the Freedom of Information Act 2000 procedures for the disposal of records must be documented. Records of disposal must be kept indicating what has been destroyed/transferred, when and by whom. This is to ensure that the organisation is aware of those records that have been destroyed and are therefore no longer available.
